Hypothyroidism is the low or decreased activity and function of the thyroid gland. It is one of the most common endocrine disorders. The thyroid gland is an endocrine gland present in the neck containing two lobes connected by the isthmus giving an appearance of the letter ‘H’. Thyroid gland function is to secrete thyroid hormone in order to influence body metabolism and protein synthesis. ‘Hypo’ in Hypothyroidism means low or less than normal activity and production of thyroid hormone. Hypothyroid condition is also called underactive thyroid.

What causes hypothyroidism?
There are different causes for Hypothyroidism to occur such as Hashimoto’s thyroiditis which is an autoimmune disease, a common cause of hypothyroidism. Certain medications can also cause hypothyroidism. It can develop during or immediately after pregnancy in relation to other changes in the body. Any surgery of the thyroid gland could lead to low functioning of the thyroid gland leading to hypothyroidism. Iodine deficiency is one of the causes of hypothyroidism. Late effects of cancer therapy can also lead to hypothyroidism
Depending on the causative factors, hypothyroidism is of 2 types
Primary hypothyroidism and Secondary hypothyroidism
- Primary hypothyroidism causes include autoimmunities such as Hashimoto’s thyroiditis and Atrophic thyroiditis. Iodine deficiency. Infiltrative disorders such as amyloidosis, Sarcoidosis, Scleroderma, Haemochromatosis
- Congenital hypothyroidism: absent or ectopic thyroid gland, Dyshormonogenesis, TSH-R mutation Drugs such as Iodine excess, lithium, antithyroid drugs, etc
Iatrogenic causes such as treatment with subtotal or total thyroidectomy, external irradiation of neck for cancer.
- Secondary hypothyroidism causative factors are Hypopituitarism; tumors, pituitary surgery, trauma.Isolated TSH deficiency or inactivity.
- Hypothalamic disease: tumors, trauma, infiltrative disorders, idiopathic.
How is hypothyroidism known?
Hypothyroidism is diagnosed by assessing the symptoms and blood tests, which measure the amount of Thyroid-stimulating hormone (TSH), T3, and T4. In the case of Hashimoto’s thyroiditis, Anti-thyroid peroxidase test is done to know whether hypothyroidism is due to Hashimoto’s thyroiditis. Clinically, Hypothyroidism is defined as an elevated TSH in conjunction with decreased T4 level.
